Transitioning from curative treatment to hospice care can be an emotional time as patients and families adjust to a new stage of care. Once a patient is admitted to Eleos Hospice, the focus shifts toward comfort, quality of life, and helping patients spend meaningful time in the place they call home.

A Team to Guide the Way

No family has to navigate this transition alone. The Eleos hospice team includes physicians, nurses, hospice aides, social workers, and chaplains who work together to support both the patient and their loved ones. From the beginning of care, our team explains what to expect, answers questions, and helps families feel prepared for the journey ahead.

Through regular visits, our team manages pain and symptoms, assists with personal care needs, and provides emotional, psychosocial, and spiritual support. As a patient’s needs change, the care team adjusts the individualized plan of care to ensure comfort and support remain at the center of every decision.

Settling in at Home

An important part of the transition to hospice is making sure patients and families have what they need to feel comfortable and supported at home. Based on the patient’s individualized plan of care, this may include a hospital bed, wheelchair, oxygen equipment, medical supplies, and medications for pain and symptom management.

Eleos coordinates the equipment, supplies, and medications needed for hospice care and provides families with the guidance they need to use them with confidence. Our goal is to make the transition as seamless as possible, allowing families to spend less time coordinating care and more time focused on their loved one.
