Hospice care brings a full team of medical and support professionals directly to your loved one, wherever they may call home. This team provides hands-on clinical care, manages pain and symptoms, coordinates medications and medical equipment, and makes regular visits to ensure each patient’s needs are met throughout their hospice journey.

At-home hospice is comfort-focused, end-of-life care delivered wherever they may call home, whether that be a private residence, assisted living community, or skilled nursing facility.

What Happens Once Care Begins?

Once a patient is admitted to hospice, the Eleos care team works closely with the patient, family, and physicians to develop an individualized plan of care based on the patient’s medical needs, symptoms, goals, and personal preferences. From there, the team coordinates the services and support needed to provide comfort and quality of life.

  • Medications and medical equipment related to the hospice diagnosis are coordinated and delivered as needed to support the patient’s comfort and care.
  • Regular visits from the hospice care team are scheduled based on the patient’s individualized needs. Visit frequency may change as those needs change.
  • Nurses and other clinicians monitor symptoms, manage pain and comfort, and make adjustments to the plan of care in collaboration with the patient’s physician and hospice medical team.
  • Social workers and chaplains provide emotional, psychosocial, and spiritual support for patients and families based on their individual needs and preferences.
  • Families and caregivers receive ongoing education and guidance to help them understand what to expect, manage day-to-day needs, and feel supported between visits.
  • Support is available 24/7 by phone when questions arise, symptoms change, or additional assistance is needed.
